Reasonable Modification of Policies, Procedures, and Programs for Disabled Individuals

Humboldt Senior Resource Center (HSRC) does not discriminate against individuals with disabilities and is committed to ensuring that no person, solely by reason of their disability, is excluded from participation in, is denied benefits of, or is subjected to discrimination under any of HSRC’s programs or activities.

HSRC will make reasonable modifications to its policies, procedures, and practices when such modifications are necessary to provide equal access for individuals with disabilities, unless:

  • The modification would fundamentally alter the nature of the service, program, or activity;
  • The modification would create a direct threat to the health or safety of others;
  • The individual is not eligible for the service, program, or activity; or
  • Without the requested modification, the individual with a disability is able to fully use HSRC’s services, programs, or activities for their intended purpose.

Examples of Reasonable Modifications

Reasonable modifications may include (but are not limited to):

  • Allowing a personal care assistant to accompany a participant
  • Assisting with mobility support inside the facility
  • Providing printed materials in large print or audio versions
  • Permitting food or drink in activity areas if needed for medical reasons (e.g., diabetes)
  • Allowing early or flexible arrival times for those using paratransit

How to Request a Modification

An individual requesting reasonable modification will be required to describe what the individual believes is needed in order to access services (s). The requestor is not required to use the term “reasonable modification” when making a request.

Individuals may request a reasonable modification in the following ways:

  • In advance (recommended): Contact the HSRC Director of Operations by phone, email, or in person.
  • During your visit: HSRC staff will do their best to accommodate on-the-spot requests whenever possible.

If a Request Is Denied

If a request is denied, the reason will be clearly explained. Individuals may request a reconsideration or file a grievance with the Director of Operations within 15 days of notification of the denial.

Grievance Procedure

If you believe you were unfairly denied a modification or discriminated against based on a disability, you may file a grievance. To do so, contact the HSRC Director of Operations.
