Redwood Coast PACE Opens in September

Redwood Coast PACE — HSRC’s new comprehensive healthcare and social services program — will open September 1. The goal of PACE is to keep frail older adults as independent as possible in their homes and community, and to delay or avoid placement in a skilled nursing facility.
To qualify, participants must be age 55 or older, need a state-certified nursing home level of care, be able to live safely in the community, and reside in our service area, which covers Scotia to Trinidad and inland to Blue Lake and Carlotta.
To enroll, or for more info, contact Matalyn Shanahan, Enrollment Representative, at 443-9747 or email .
